When you are finished entering information click on the Submit button. Please only click once. To clear the form, click the Reset button. The next page will be a confirmation page of what you have entered. If you have not entered required information, you will be prompted to return and complete the necessary information. If the information is complete, we will email you, usually within 48 hours, a quote for your finished custom box, along with payment options and instructions. Otherwise we will send you an email requesting any other information we may require to provide you a quote.

Disclaimer: If any graphic, logo, picture, design, artwork or other image is submitted by the customer for application to a Millennia Wood product, by such submission, the customer makes claim to the rights of that image and agrees to hold harmless Millennia Wood from unintentional copyright infringement.

Money Back Guarantee. If you are not satisfied with your Millennia Wood purchase, return the items in their original condition, to Millennia Wood within 30 days of receipt, for a full refund less shipping/handling and any scanning or set up fees. Unfortunately, we can not extend a full money back guarantee on items with customer-supplied images.
